A contact form

Each email to the FIFO's address goes to your inbox, and you can reply to the sender. For a web page with a form, add an ingress envelope with the same lines and a free name in To, and write a note about it.

A form with a thank-you page

Put a form on your site with the fields email and message, and the FIFO's address in its action. Each message goes to your inbox, and the browser moves on to the thank-you page of your site. Only the form on your site gets this redirect: the browser tells mkfifo.com which site sent the form, so a link to the FIFO cannot take anyone to a different site.
